The Unity House Young Adult/MICA Case Management Services Program is looking to hire a Case Manager III. Through a network of support services, home and field visits, daily living skills training, symptom management and social skills development, services are tailored to meet individual needs. Responsibilities include but are not limited to: engaging in ongoing, regular interactions and providing supportive counseling with program consumers in a manner that is in keeping with agency mission and service philosophy. Crisis prevention and intervention, assisting consumer with identifying and obtaining goals, transportation and advocacy. Required: Bachelor’s degree in health, human or educational services, and one year qualifying experience; or Associate’s degree in health or human services or certification as an R.N. or L.P.N. and two years qualifying experience. Must have reliable transportation, current NYS driver’s license and acceptable driving record as determined by our company’s liability insurance policy. Access to reliable vehicle required. Must be able to work a flexible schedule which may include holidays, evenings, weekends, and overnight.
