Provides outreach, engagement and supportive care management services to eligible clients enrolled in New York’s Medicaid Health Home program. Assist consumers to manage one or more chronic and/or mental health issues, coordinate care between providers, promote primary and preventative care to avoid emergency room visits/hospital stays. Bachelor’s degree or equivalent experience preferred, proficiency with web based programs and Microsoft office required. Valid NYS driver license in good standing and access to a reliable vehicle is required. Experience with Medicaid delivery system documentation requirements and professional writing skills preferred. Two positions available, one being bi-lingual in English/Spanish.
