  Job Description

Health Educator

Care for the Homeless (CFH) is a growing not-for-profit health care and social service agency that serves the homeless population of New York City.  CFH operates Susan’s Place, a homeless residence and medical clinic, and provides medical and social services at thirty shelters, soup kitchens, and drop-in centers.

SUMMARY
This position is responsible for screening and assessing the educational needs of our patient population and their families and for planning, implementing and evaluating effective methods to meet those needs. The position also involves developing and organizing materials for staff and coordinating all community outreach and educational programming, including lectures, classes and workshops. In addition, HE will provide one-on-one health counseling to clients.     

This employee will work as part of the PCMH team with the following essential duties and responsibilities:

  • Perform a needs assessment in assigned sites to determine programming and interventions;
  • Conduct outreach to clients in assigned sites to facilitate their participation in on-site medical, health education and social services;
  • Develop and maintain relationships with shelter staff at medical sites and medical professionals including: medical, mental and dental providers, and social services, case managers and social workers;
  • Serve as a link between shelter staff and medical clinic.  Provide staff with on-going updated information about interventions, programming and promotions;
  • Provide HIV/HCV counseling, referral, and partner notification services to clients;
  • Perform HIV/HCV rapid testing along with confirmatory results.
  • Provide linkage to care for clients reporting HIV/HVC positive through CFH and/or outside referrals.
  • Documenting all activities using agency’s data collection method ECW.  In addition, entered required data into NYCDOH database, eSHARES
  • Facilitate health education workshops and trainings for clients and staff on topics that may include sexual health, oral health, nutrition and chronic diseases;
  • Create/order educational materials for trainings;
  • Act as a resource person in health education to clients and staff and educate clients on their health care rights;
  • Serve as an active team player in health fairs, health promotions and other team events;
  • Attend and participate in meetings, trainings and staff development; and
  • Perform other duties as it relates to health education

JOB QUALIFICATIONS

Educational Requirements:  

  • Bachelor’s Degree required in Health Education and Health Promotion, Community Health, or Public Health.  CHES certification preferred. 

Experience:

  • 2 years prior experience working as a Health Educator in a medical clinic setting. 
  • Prior experience or training in health education
  • Prior experience in developing, implementing and evaluating programs
  • Prior experience in creating interventions for chronic illness

Skill Sets:

  • Strong organizational skills need to manage diverse responsibilities and provide services at various sites.
  • Ability to establish and maintain effective working relationships with colleagues, clients and contracted providers of health care services.
  • Ability to plan, organizes, implement, and evaluate needs assessments; action plans; and professional development activities.
  • Apply problem-solving skills to resolve issues or concerns regarding HIV/AIDS prevention education and health education services.
  • Compassion/understanding/desire to work with homeless individuals.
  • Excellent oral and written communication skills needed to provide counseling to clients, document case work and provide training.
  • Ability to work independently and within a team environment.
  • Ability to adhere to confidentiality guidelines.

Languages: 

Bilingual English/Spanish strongly preferred but not required
