Community Health Councils (CHC) is a non-profit, community-based health education, advocacy and policy organization established in 1992 in response to the growing health and healthcare crisis. CHC’s mission is to collectively build equitable systems. CHC has been on the cutting edge of public policy and program development, working to strengthen community participation and inform state and local policies that: (1) improve community health and education to achieve optimal health and healthy lifestyles; (2) ensure equitable access to quality healthcare services; (3) provide universal access to quality healthcare coverage and (4) eliminate health disparities. CHC engages members of impacted communities in program and policy development through a wide variety of community interventions and strategies as well as representation and leadership through the coalitions and collaborations convened. With a primary focus on South Los Angeles, CHC works at the local, state and national level to improve the health of under-served and marginalized communities.
CHC’s policy work is achieved through policy research, local coalition’s coordination, and a select number of demonstration projects that aim to establish practice and evidence to support our policies. The focus of this work is to address health disparities and health equity, starting with South LA as our geographic center. The Project As/Navigator works under the Accessing Benefits for Children (ABC) Project, which is funded through the Los Angeles Department of Public Health Children’s Health Outreach Initiative (CHOI) . The ABC Project seeks to improve health conditions and reduce the number of uninsured children and families in Los Angeles County Service Planning Area 6 and parts of 8 (Inglewood, Lennox, Gardena and Hawthorne). This position reports to the Policy and Community Engagement Manager but will also work under the direction of Human Resource Manager.
This position is responsible for supporting the internal team with quality control and data management by reviewing data entered into the County’s CHOI Database, maintaining project files, supporting the development of monthly reports, maintaining project supplies and incentives, and collaborate with the team to identify, schedule outreach activities and provide application enrollment assistance and follow-up for agency walk-ins. This position will also support the front office and direct clients to appropriate staff.
