The Appointment Coordinator will be responsible to accompanied individuals to their medical appointments and communicate effectively with medical professionals to coordinate the best possible care. Manage medical appointments to ensure compliance. Assist House Manager and Program Manager with gaps in schedule.
1. Implement nursing plans.
2. Accompanies supported person to medical appointments
3. Ensure accurate documentation of medical services.
4. Ensure that medications are administered and managed according to policies and procedures.
5. Ensure that medical and mental health appointments are accomplished in a timely manner.
6. Handles medical appointments as assigned. Arrives for appointments on time, with appropriate information about the individual’s medical history and current condition.
7. Communicate effectively with medical professionals to coordinate the best possible care.

Individual Plan Implementation and Maintenance
1.Reviews plans and demonstrates familiarity with each individual’s plan
2.Implements each individual’s plan as prescribed and asks for clarity if necessary.
3.Records data on outcomes and Target Behaviors accurately and promptly.
4.Demonstrates familiarity with Behavior Support Plans and implements plan in a consistent manner.
1.Demonstrates good judgment when reacting to incidents or resident injuries.
2.Follows the agency’s policies and procedures for reporting incidents.
3.Completes incident reports completely, correctly, and promptly.
1.Provides instruction, guidance, follow-up, monitoring, and assistance, when necessary, in personal grooming and hygiene.
2.Assists with and/or dispenses medications and other medical treatments to the individual as necessary
Communication and Documentation
1.Uses effective, respectful, and frequent communication with the Program Coordinator or Manager.
2.Documents and completes required records and reports in a clear, consistent, and legible manner.
3.Submits paperwork that is complete and legible by prescribed due dates.

Must have a high school diploma. Have at least 1 year of related experience working with children or adults in a care-giving setting. Must have a good driving record, reliable vehicle and valid current Driver’s license for a minimum of 2 years with no more than 2 points on the license. Be able to successfully complete training requirements of The Arc.
Mental, Physical, and Visual Demands
Must respond professionally to stressful situations. Employee will have to drive a company’s van and be able to lift.
Works in the residential home and must be able to respond to emergencies effectively.
