Job Description

Position Summary:

Orchestrate teams to work together to deliver effective and efficient individualized services and supports, customized for each persons unique needs and dreams to live a full life in the community. State of the art activity and teaching designed to enhance meaningful life experiences and contribute to growing independence in all areas of living. Supports and services offered reflect optimal quality in a culturally sensitive, customer-focused manner.

Qualifications:

  • Bachelors degree in Social Services or related field; Masters preferred.
  • QIDP certified or eligible.
  • Two years supervisory experience.
  • Five years experience working with people in independent living (unless Masters degree); developmental disabilities preferred.
  • Valid Drivers License with good driving record.
  • Minimum 21 years of age.
  • Effective oral and written communication skills; ability to write reports and public speaking.
  • Ability to effectively present information and respond to questions from families, employees, and the general public.
  • Ability to maintain confidential information.
  • Ability to deal tactfully and courteously with individuals, employees at all levels, families, and the general public.
  • Ability to work flexible hours and days, including some weekends and holidays.
  • Ability to be on-call to direct reports and supervisor.

Position Responsibilities:

  • Supervise and teach direct reports by providing documented supervision to assure total quality, agency, and departmental goals are met, including hiring, training requirements, scheduling, talent development reviews, disciplinary action (in conjunction with the supervisor), etc.
  • Ensure that individual and life consultant work together to complete each goal in accordance with LoMO Individual Service and Health/Wellness Plan.
  • Ensure compliance with safety, health, and individuals rights; maintain consistent direction and integrity. Oversee that each individual is treated with dignity and respect, given the opportunity to exercise his/her individuality, and has access to explore opportunities of interest.
  • Ensure that individuals goals are completed with the team members, ensuring expected standards of care and quality of services along with appropriate documentation is completed.
  • Ensure weekly activities and outings are discussed with individuals and are appropriate for growth, learning independence, and fun.
  • Effectively communicate within the appropriate timeframe all necessary information to the proper team members, departments, and others who need to know. Maintain a consistent and professional line of communication with guardians and/or family and friends of each individual.
  • Assure proper Support & Case Management/individual ratios according to the needs of the people we serve; Monitor payroll and use of hours in assigned department to include overtime, time off, supervision, team meetings, etc.
  • Oversee and management of allotted home/program budgets; monitors monthly along with other financial statements of department; takes effective corrective action or makes recommendation to supervisor as required.
  • Provide Administrative on call Monday Sunday to direct reports, unless reassigned and approved by Vice President. Response time is within 15 minutes. Best practice: appropriate documentation of all calls is recorded.
  • Complete all necessary reports, forms, billing, and documentation accurately and timely (program, human resource, DHS, Proviso, etc.).
  • Make recommendation and assist with necessary changes in development and delivery of new and/or existing policies, procedures, services, and programs to optimize quality assurance and reduce deficits in all programs, and maintain the integrity of accreditation, licensure, and all funding guidelines/contracts.
  • Actively recruit for candidates that will be successful in the LoMO program. This includes tracking of all clients and patterns in admissions
  • Oversee all billing and invoices for the program insuring accuracy and timeliness.
  • Oversees all quality measures set for your team members, programs, and adjusts to meet set outcomes for department.
  • Performs other duties as directed by supervisor within the scope of the position along with actively participating in professional development.
