The Care Manager reports to the Quality Department, and is responsible for coordinating referrals and linkages to clinically indicated services. The Care Manager’s role is helping members navigate and access community services, other resources, adopt healthy behaviors, improve health outcomes, and reduce healthcare costs. This role supports providers and Managed Care Coordinators through an integrated approach to care management and community outreach. The Care Manager supports the Managed Care Organization (MCO) and implements program improvements that strengthen care management, member access, and health outcomes.
