About the Organization:

The mission of New Moms is to express the love of God by surrounding young moms and their children with all they need to transform their lives. New Moms is a 34 year old nonprofit organization, with a budget of $3.1 million and a staff of about 50, focused on serving young moms in Chicago. New Moms’ model provides a comprehensive range of services to young moms and their children including supportive transitional housing, workforce development, and early childhood services. New Moms’ headquarters is located on the west side of Chicago in the Austin neighborhood. Our social enterprise, Bright Endeavors, which provides paid transitional jobs for young moms, is located nearby in the West Garfield Park neighborhood. We also have another location in Oak Park.


About the Job:

A part of the Home Visiting for Homeless Families Demonstration Project, this specialized early childhood home visitor will work exclusively with homeless families in Chicago and near western suburbs by implementing the evidence-based Parents as Teachers (PAT) curriculum and provide case management interventions in coordination with other homeless service providers. S/he will have significant travel around the city in order to provide early childhood services and support to an important, yet underserved, population of homeless parenting youth. Salary: $38k


Job Responsibilities:

  • Maintains a caseload of 8-12 participants (ages 24 and younger at intake) and their children as part of this initiative.
  • Leads the effort in recruiting, screening, and intake of participants for this demonstration project.
  • Conducts thorough initial assessments of participant families and utilizes prescribed assessment tools on established schedule.
  • Develops written service plans, with regular updates, for each assigned case including participant and children, specifying goals to be achieved and services to be provided.
  • Conducts home-based visits with participants that consists of approved, age appropriate parent child activities and observations intended to enhance the parent child relationship and uses the PAT model. Service needs, referrals and goals progression are also discussed during the visit. Performs crisis prevention and other case management interventions as needed.
  • Assists with transportation of participant and children as required and approved by supervisor.
  • Works in tandem with other New Moms staff to integrate services across the agency; collaborating with staff and other service providers to ensure continuity of care and service delivery.
  • Maintains working relations with other social service or public agencies; providing linkage of participants to appropriate resources and advocates for participants as necessary. Secures appropriate release and consent forms and facilities inter-agency referrals as needed.
  • Assists with weekly parent support group meeting as needed.
  • Performs written and electronic recordkeeping functions as required and in a timely manner.
  • Works to consistently enforce program compliance under the direction of the program supervisor, working to maximize participant retention as well as program safety.
  • Coordinates discharge planning, resources and referrals.
  • Actively participates in weekly supervision, team meetings, and agency staff meetings.
  • Regularly participates in agency or professional group seminars or conferences to enhance professional knowledge and job skills.
  • Other duties as assigned.


The successful candidate will posses:

  • Enthusiasm for mission-driven work and interest in work with young moms at the time of pregnancy and early parenting.
  • Master’s Degree in the area of social work, child development counseling, or psychology preferred; Bachelor’s required.
  • Parents as Teachers (PAT) certification preferred.
  • 2+ years case management and crisis intervention experience working with vulnerable populations, preferably with early childhood through adolescent parenting, child/family development and urban issues.
    • Endorses a Family-Centered, Trauma-Informed, Strengths-Based practice model that integrates harm reduction, motivational interviewing, executive skills, infant mental health, and positive youth development practices in working with young moms and their children required.
  • Position requires utmost integrity; ethical behavior and appropriate business practices are expected.
  • Strong interpersonal and communications skills, and ability to work both collaboratively and independently.
  • A functional car with valid insurance, along with a valid driver’s license.
