About Flexible Services
In 2018, The Commonwealth of Massachusetts received a five-year waiver from the Centers for Medicaid and Medicare to improve the ways in which MassHealth provides care to its members. Among many other elements, includes funding for a pilot Flexible Services Program. Through the Flexible Services Program, accountable care organizations (ACOs) may pay for health-related social needs like nutrition and housing supports.
Project Bread has partnered with multiple ACOs. Through these partnerships we receive referrals from community health centers and primary care practices for individuals who are both food insecure and have a medical condition. Upon receipt of referrals Project Bread staff conduct assessments and develop individualized service plans.
To assist us in meeting our mission, Project Bread is seeking a Nutrition Services Coordinator to join our team.
Supervisor: Director of Health Care Partnerships
Supervises: N/A
Role: The Nutrition Services Coordinator will respond to referrals. The Coordinator will develop service plans customized to meet the individual needs of each client so that they can access the healthy foods needed to best manage their health conditions. This position will primarily work with members remotely, but the role will include in-person visits on an as needed basis (post-coronavirus).
To ensure that the team of Nutrition Services Coordinators can best meet the needs of our clients we seek individuals who speak the same languages as those who are referred to the program. Therefore, we are currently hiring a Coordinator who is fluent (written and verbal) in Spanish.
