  Job Description

The Foster Care Social Worker will provide a comprehensive range of social work and therapeutic services to support children and families by serving as an advocate, a liaison to other human services and educational agencies, and a coordinator of supportive social work and mental health services.

Responsibilities:

  • Provide social work support and therapeutic consultation to youth in treatment foster care, foster families, and biological or adoptive families.
  • Develop strengths-based treatment plans with a collaborative team involving the youth, family, and foster parents, and utilize a variety of treatment approaches with clients.
  • Coordinate therapy and mental health treatment services with the team.
  • Liaison with collateral social work services and identify appropriate resources to assist families and youth with meeting mental health treatment objectives.
  • Bridge relationships with foster parents and biological families and provide role modeling.
  • Provide timely and accurate communication to supervisors and colleagues regarding significant events to esure continuity of care.
  • Supply documentation of social work, treatment, and coordination activities.
  • Work with the team to develop a discharge plan that supports the youth and family's continuing relationship-building, independent living skills, and mental health progress.

Qualifications:

Candidates for consideration must meet the following minimum requirements:

  • LGSW (Licensed Graduate Social Work) or LCSW (Licensed Clinical Social Work) license required.
  • Valid driver’s license required. Must meet state regulating agency driving requirements, maintain auto insurance and pass corporate insurance eligibility standards. Must be able to spend a considerable amount of time driving to client locations and transporting clients as needed using one’s own vehicle.
  • Must be able to walk, sit, stand, crouch, twist, bend, reach overhead, maneuver stairs and stretch in a manner conducive to the execution of daily activities for a social worker visiting clients in their homes.

Qualified candidates should possess the following:

  • Experience working in the social services or mental/behavioral health fields.  Experience with foster care is a plus.
  • Exhibit clear, professional, respectful and effective communication when interfacing with clients, coworkers, families and external contacts and willingly share knowledge, information and resources appropriately.

  • Demonstrate sufficient written communication skills to generate required documentations and reporting that is legible and articulate.

  • Demonstrate concern and empathy towards others as needed, while maintaining appropriate boundaries.
  • Interact with and appreciate people from diverse cultural, social, economic and religious backgrounds.
  • Maintain a schedule that is adjustable and flexible to meet changing work needs and demands of clients and their families.
