Job Description

 

New Destiny’s mission is to end the cycle of violence for low-income families and individuals experiencing homelessness and domestic violence by connecting them to safe, permanent housing and services. To achieve its mission, New Destiny builds and manages housing with on-site services; offers innovative programs that assist survivors in finding and retaining affordable housing and expands access to permanent housing resources for survivors.

New Destiny, in partnership with the NYC Department of Housing Preservation and Development (HPD) and the Mayor’s Office to End Domestic and Gender Based Violence (ENDGBV), is assisting New York City in administering the Emergency Housing Voucher (EHV) programs for households fleeing or attempting to flee domestic violence (DV) and is seeking an Aftercare Coordinator to support this project. The Aftercare Coordinator will be tasked with providing support to survivors following placement into permanent housing. The majority of placed individuals will need light touch support and/or warm referrals to other providers. However, given the large number of placed individuals from diverse backgrounds, New Destiny expects that a small cohort will need more in-depth clinical care. The Aftercare Coordinator will triage these individuals and refer them directly to a clinical provider that is partnering with New Destiny.  

The qualified candidate will have the following responsibilities:

Aftercare Services

  • Conduct ongoing monthly check-ins with placed individuals to help them remain stably housed
  • Assist participants in identifying supports or programs that can aid them increasing their financial security, and make referrals to said organizations
  • Provide referrals and linkages to case management and other services for individuals who need support due to rent arrears, safety concerns or other issues that may jeopardize their safety.
  • Identify and triage placed individuals in need of more comprehensive mental health care management to the clinical partner.

Data Management

  • Maintain individuals’ data integrity by inputting and updating participant information, such as assessments, case notes, and demographic data, to New Destiny’s case management database in real time.
  • Work with supervisor and the Data and Evaluation Manager to identify trends in program participants’ needs or gaps in services.
  • Independently review internal progress reports to determine participant’s progress toward annual goals.

Partnership Management

  • Coordinate with Family Justice Center (FJC) staff and supervisor regarding additional service needs and to conduct case consults as needed
  • Maintain and establish new relationships with other community-based organizations.

The ideal candidate will:

  • Have at least 3+ years experience working in the fields of domestic violence or related social services and bachelor’s degree in social work or related field. Lived experience with domestic violence and/or housing insecurity will be considered equally to professional and educational experience
  • Have professional proficiency in English and one of the ten designated citywide languages (preferred). The ten designated citywide languages are Spanish, Chinese, Russian, Bengali, Haitian, Korean, Arabic, Urdu, French, and Polish.
  • Be responsible, flexible, a self-starter and able to work independently
  • Be an excellent oral and written communicator
  • Have experience in interacting with people from diverse backgrounds
  • Be familiar with local and federal housing subsidies (CityFHEPS, FHEPS, Section 8, etc.)
  • Be comfortable traveling within NYC as needed to provide support to program participants
