The Case Manager plans and coordinates treatment and services for youth and young adults with mental health needs.

The Case Manager serves as the primary transition facilitator by providing the coordination of treatment, treatment planning and the development and implementation of individualized service plans for youth and young adults with mental health needs. The case manager facilitates services – like school, medical providers, family, etc. – and also assists in skill building through coaching and one-on-one work with young adults.
