Job Description: The AFC Community Health Nurse works as part of a collaborative team in the Adult Family Care Department. S/he participates in determining client eligibility for services based on nursing assessment data. S/he provides periodic monitoring of appropriateness and quality of care authorized. Maintains collaborative relationships among community agencies and institutions and acts as in-house consultant regarding the health issues of aging.
QUALIFICATIONS: Graduation from a school of nursing accredited by the National League for Nursing required; BSN preferred. Valid RN license in Massachusetts required. Minimum of 3 years clinical experience; with one year in community health required. Previous experience with elder and/or disabled population essential. Excellent communication and presentation skills required. Knowledge of the home care system and community resources preferred. Spanish speaking preferred. Vehicle required

Compensation: Low to Mid $60’s for a 35-hour week coupled with a generous benefits package including medical, dental, life, short and long term disability, 401(k) with agency contribution, 13 holidays and 24 days of accrued earned leave.
