The Community Health Nurse/AFC Program works as part of a collaborative team to support AFC placements. S/he participants in determining client eligibility for services based on nursing assessment data. S/he provides periodic monitoring of appropriateness and quality of care authorized. Maintains collaborative relationships among community agencies and institutions and acts as an in-house consultant regarding the health issues of aging and the disabled. No nights, weekends or holidays.
Graduation from a school of nursing accredited by the National League for Nursing required; BSN preferred. Valid RN license in Massachusetts required. Minimum of 3 years of clinical experience required. Previous experience with elder population and/or disabled essential. Excellent communication and presentation skills required. Vehicle required. Spanish speaking preferred.
High $50s for a 35 hour work week with a 4-day week option coupled with a generous benefits package including medical, dental, life, short & long term disability, 401(k) with agency contribution, 13 holidays and 24 days of accrued earned leave.
