The Transition Facilitator is responsible for carrying out the daily routines of an independent living program for LGBTQ youth on parole and probation, ages 18-24, who are re-entering the community from incarceration or a structured residential settings and in addition will provide case management services to assigned youth. The position is responsible for assessing Independent Living Skills Levels for all clients entering the program, as well as coordinating the delivery of this work, to include working with the team to identify the appropriate work and to assist the client in achieving mastery of independent skills.
This position is also responsible skill development and crisis intervention for up to 12 clients at any given time. It will monitor and follow through with clients regarding their case plans, in part through the delegation of tasks to the Life Skill Coordinator and Mentors. This could include crisis management while still covering the floor and attending to the safety of other clients as well as tracking community clients all at the same time. Therefore it requires extensive exercise of independent direction and judgment as the Rivera House typically is only staffed with two staff per shift.
SCHEDULE: Sunday 8am to 4pm, Monday 8am to 4pm, Tuesday 1:30pm to 9:30pm, Wednesday 1:30pm to 9:30pm, and Thursday 7:30am to 3:30pm. However, this schedule may be adjusted according to the needs of the program.
