Case Manager - Community Support Program 1 - $14.40/hr Closing Date: 7/16/18
F/T 40hr/wk (Temporary) position for busy and enthusiastic case management program supporting people in reaching goals. The Community Support Program provides the appropriate level of support necessary for members to remain in the community. Duties include assisting members in developing goal plans, guide members in developing skills to enhance quality of life and provide links to needed or requested services as determined on an individual basis. Worksite: Norwich Hours: Monday-Friday, 1st Shift, (this is a Temporary position last about 4 months).
Duties and Responsibilities:
Implementation of program and agency policy and procedures
Participation in the service plan process including creating initial IRP, crisis plan, risk assessment, as designated and in collaboration with director and clinician as needed.
Attend team meetings as designated by the Director
Provide rehabilitation, basic support, case management, and crisis interventions per member service plans
Provide required documentation of services within the deadlines of the Agency.
Report on residential status to Program Director and staff.
Provide input to team decisions regarding admission, discharge, and counselor assignments.
Assist members in maintaining apartments and landlord standards when applicable.
Assist members in financial matters as needed.
Provide transportation related to services when necessary
Monitor member compliance to their conditional release order when applicable.
Required to provide coverage as needed at any of the Agency sites per Agency need
Engage in continuous concerted efforts to access, obtain, respond to pertinent agency information/communications and disseminate as appropriate
Provide conditions that maximize safety for co-workers and program participants.
Other responsibilities of Reliance Health staff, such as participation in Agency events, adhering to Agency Policies, training requirements etc.
