  Job Description

Somerville-Cambridge Elder Services promotes the right of all individuals to live with dignity, in the setting of their choice by offering older people, younger people with disabilities, and caregivers the information, services and support needed to make choices which enhance health, well-being and independence. Since 1972, Somerville-Cambridge Elder Services (SCES) has been the area's aging information and service center. SCES helps older people and people living with disabilities remain safe and independent in their own homes by providing a wide range of supportive services, as well as information and advice.
 The Community Care Coordinator is hired by the ASAP to work with individuals and caregivers within the Cambridge Health Alliance primary care settings and will serve as a team member on the clinics’ Complex Care teams.  The CCC will work with individuals who meet the criteria of High Utilizer, as defined by CHA, with the goals of:  decreasing hospitalizations and ED visits, making care more streamlined and efficient, increasing patients’ quality of care.    The CCC will do this by assisting patients in transitioning from hospital to home, assessing patients in their homes to identify gaps in services and making referrals as needed, and serving as a bridge between the patient, the primary care practice, acute care and the community

Requirements

  • Bachelor’s degree

  • Bi-lingual preferred

  • Two years of experience in individual needs assessment and care planning, with a focus on eldercare issues and resources.

  • Ability to be a self-starter, work independently, and demonstrate flexibility in responding to consumer needs.

  • Ability to be flexible and be able to adapt to, and work effectively in, the culture of a medical environment including patients.

  • Excellent interpersonal, organizational, time management and customer service skills.

  • Excellent computer skills.

  • Excellent written and verbal communication skills.

  • Must be willing and able to use own vehicle

     

    Compensation: Mid $45 to Low $50K
