Provide housing placement and supportive services to those living with HIV/AIDS. Program will coordinate care and work closely with other providers to help promote stable housing and access to medical care to improve clinical outcomes. Responsibilities include engaging in ongoing, regular interactions with program consumers in a manner that is in keeping with agency mission and service philosophy, ensuring housing is up to quality standards, landlord advocacy, completing compliance activities, producing final reports on outcomes as required. Requirements include previous case management experience; experience with multiple populations preferred. Must have reliable transportation, current NYS driver’s license and acceptable driving record as determined by our company’s liability insurance policy.
