The Unity House Health Homes program is looking to hire a Care Manager. A Health Home is a care management service model whereby all of an individual’s caregivers communicate with one another so that all of a patient’s needs are addressed in a comprehensive manner. Responsibilities include but are not limited to: Provides outreach, engagement and supportive care management services to eligible clients enrolled in New York’s Medicaid Health Home program. Assist consumers to manage one or more chronic and/or mental health issues, coordinate care between providers, promote primary and preventative care to avoid emergency room visits/hospital stays. Requirements: Bachelor’s degree or equivalent experience preferred; Proficiency with web based programs and Microsoft office required. Must have reliable transportation, current NYS driver’s license and acceptable driving record as determined by our company’s liability insurance policy. Prefer experience with Medicaid delivery system documentation requirements and professional writing skills.
